Same problem here. Problem still exist with latest update. 12 Oct 2017. When Advanced Formats on, with or with any code, problem coming out. e.g. Select 2 November 2017, it become 2017-2-11, (wrong) but if select 31 November 2017, it become 2017-11-2 (correct)

After apply latest github, problem solve. Thanks.

I also have have this problem, after I delete browser cache, clear joomla cache, the add icon coming back.
