Am I looking at the right site?
The site you have in your My Sites, the 'soulkeeper' element on the 'soulkeeper' form is a dropdown, not a join element:

... and there isn't a list or a table called 'add_sk'.

Well, that's why I couldn't see anything, wrong site.
OK, I've fixed the code, it now runs. I don't know if it does what youw ant it to, as you are trying to add to a list ID 6, which doesn't exist, so you'll need to take a look at that.
As per my previous response, you had sk_add in the select, not add_sk.
