Upload error

Any ideas why I'm getting this error?

The upload is working for some people, but not for others. Same form....
It is NOT using Ajax for uploading.
I am using custom folders and the folder is created..but no images are in the folder.
images/canaldays/2013/vendors/{festos_vendors___vendorName}
