Clear form fields after submit

After form submit (to email) the fields are not cleared and even though the success message is shown, it looks like it didn't send because all of the fields still contain the data entered by user. Is there any way to clear the data after submit action? Joomla 3.6.2/Fabrik 3.5

Also, using the default form (I didn't assign a php form), I can't get the fields to auto-populate. I've tried setting the 'auto complete' setting to both 'browser' and 'values' options but I have to enter the field info every time.
