Form display in Module

I am using fabrik module to display a form. At the end of the form I am getting a line

"FabrikControllerForm".

How I can hide this line?

Thanks Troester for pointing me to that thread.
But Now I have noticed a bigger problem. While testing the form in the module, I can't submit the form. On clicking the submit button, nothing happens.
