Ajaxified Email List plugin not loading correctly in front-end

I'm trying to use the email list plugin to email records. The plugin seems to work great in the backend, but it does not load correctly in the front end. When I click the email icon, the popup looks like this:

Something seems to be wrong with the javascript when I run it with my template. The template I am using is Shape5 Forte if that makes any difference.

So I found out that the problem only occurs because I have the same list displayed twice on the same page. I used the content plugin to filter a couple things from each list. I fixed the issue by copying the list and creating a prefilter for each new list. I then called the new lists separately with the content plugin.
