Custom form template example

Hi Guys,

I want to create a custom form template. In the wiki (http://fabrikar.com/forums/index.php?wiki/form-and-details-templates/) you state to copy a custom contact us template. But in the new fabrik (did fresh install) the custom contact_us template isn't there to copy anymore and the old one gives an error.

Is there a simple example somewhere i can use, or can you put in a new custom template example?

Please find attached :

1. one example of a custom form template (default.php file) called form-default.txt (folder : components/com_fabrik/views/form/tmpl/templatename)
2. one example of a custom details template (default.php file) calleddetails-default.txt (folder : components/com_fabrik/views/details/tmpl/templatename)

It is quite self explanatory.
