Difference between new and edit items

I am struggling with a weird problem.

If I edit a record my template loads correctly.

If I create a new record, my template borks. I dont think its the template as I havent made any changes.

Has anyone seen this before?

I haven't seen this exact problem before.

I can only ask the usual, sounding like a stuck record, are you running the latest github?
