Form Permissions - table set to everyone, but will not show the fields

Good afternoon,

I am trying to produce a form that everyone can see and provide information from. It is visible for those who log in, but not to those who do not. I want anyone who visits the site to be able to send me information using this form, but it is not happening.

Here are some screen dumps of the problem. Can you help please.

You should leave the Elements access to default (Everyone / Everyone) and just change the Access on the form.
But if you want everyone to send you data using the form you should set 'Add records' to Everyone. And if you don't want anyone but admins to see the records (the table with all records and the records' details) you should change View Table and View Records to Admins. (Edit Record to admins too probably).
