updated, but i'm still getting this exact issue. tried stopping and restarting browsers, and using FF and chrome.
happens when using the advanced dates = yes or no.
click on the link above and you can see the js error. or click the caln button (which functions as a 'submit' sans js).
note: i update using wget --no-check-certificate https://github.com/Fabrik/fabrik/tarball/joomla3 -O - | tar -zx --strip-components 1

Should be fixed on your site and in github. It was an issue with muliple date elements, some using the advanced option and others not using it. If the element not using advanced date formatting was last in the form, then it would removed the advanced date js files from the shim we use to load up the form's js files. This is now fixed.
