How to link to a form from a module

I am trying to link to a Fabrik form from a module using Joomla Weblinks. It works if I put the URL in the link, such as
http://www.site-name.com/index.php/members/members-renewal
Problem here is the user must be logged in to use the form, and if he is already logged in when the link is executed, the system doesn't know it and asks him to log in. Not very user friendly.
I've tried things like
index.php?option=com_fabrik&view=form&fabrik=36&random=0
and that gives a message the form is not published.

I don't want the form displayed in the module as the Fabrik form module does.

I haven't been able to find a clue that works from the forum, wiki, or Google.

Appreciate any help.

If you are using fabrik 3 then the url should be:

index.php?option=com_fabrik&view=form&formid=36

To be honest I'm not 100% sure about the rest of your post, if there's anything else you need from us?

Thanks, that does it. Too bad there so much mis/obsolete information floating around.
