Multiple items on form

I have a form where it will be something like

Service requested ______

Name age sex
Person 1 ____ ___ ____
Person 2 ____ ___ ____
Person 3 ____ ___ ____



What is the best way to get a form to be laid out something like that?? I think it has something to do with groups. Do I have to build a custom template???

Put "Service requested" in one group.
Put Person stuff in another group. One it is working for one person, set this group to repeat.
