I have installed fabric 1.0.5 in joomla 1.0.14, all seems to be fine, I have publish contact form example in the main menu and when I click to menu I cant see the form, just a blank page, I have try publishing in static content using the mambot and the same result, blank page.

Have you checked that the form and table are both published in Fabrik, and that the access levels are set properly (on the Table settings).

One thing you may consider, if J! and Fabrik both are stopped with a complete white screen, PHP probably stopped with a fatal error.
I've seen this if there wasn't enough memory allocated for PHP to pull from.
If you have access to your server error log, look in there, you'll probably see the PHP Fatal error .... etc.

What he said. And maybe up your error reporting level in php.ini, see if that throws up anything interesting.
